Health Disclaimer and Liability Waiver

1. General Health Disclaimer

The services provided by Sensa Spa LLC, including but not limited to bathhouse access (thermal circuits, steam rooms, cold plunge pools, rain room), massage therapy, medspa treatments, IV drip therapy, GLP-1 and peptide therapies, red light therapy, and related wellness services, are intended for general wellness and relaxation purposes. They are not a substitute for professional medical advice, diagnosis, or treatment.

2. Assumption of Risk

By using our facilities and services, you acknowledge and accept that: exposure to extreme temperatures (hot and cold) carries inherent health risks including but not limited to cardiovascular stress, dehydration, dizziness, fainting, and in rare cases more serious medical events; medspa procedures involve risks including allergic reactions, bruising, swelling, infection, and unsatisfactory results; IV therapy and injectable treatments carry risks including but not limited to bruising, vein irritation, allergic reactions, and infection; and physical activities and treatments involve inherent risks of injury.

3. Health Conditions

You should consult your physician before using our services if you: are pregnant or nursing, have a heart condition or cardiovascular disease, have high or low blood pressure, have diabetes, have epilepsy or seizure disorders, have a compromised immune system, are taking blood thinners or other medications that may be affected, have had recent surgery, have skin conditions or open wounds, have any chronic illness or medical condition, or are under 18 years of age.

4. Health Intake Form

All guests are required to complete a health intake form before their first visit. You are responsible for providing accurate and complete information. Failure to disclose relevant health conditions may result in increased risk and releases Sensa Spa from additional liability. You must inform our staff of any changes to your health status at subsequent visits.

5. Medspa Services Disclosure

All medspa treatments are performed under the supervision of a licensed medical professional. Individual results vary and are not guaranteed. A consultation is required before certain procedures. You will receive specific consent forms for medspa treatments that outline the risks, benefits, and alternatives for each procedure.

6. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY FLORIDA LAW, SENSA SPA LLC, ITS OWNERS, OFFICERS, DIRECTORS, EMPLOYEES, CONTRACTORS, AND AGENTS SHALL NOT BE LIABLE FOR ANY INJURY, ILLNESS, DAMAGE, OR LOSS RESULTING FROM YOUR USE OF OUR FACILITIES OR SERVICES, EXCEPT WHERE SUCH INJURY OR LOSS IS CAUSED BY OUR GROSS NEGLIGENCE OR WILLFUL MISCONDUCT.

7. Digital Waiver

All guests will be required to sign a digital liability waiver upon check-in at our facility. The waiver must be signed before accessing any services or facilities. A parent or legal guardian must sign on behalf of any guest under 18 years of age (where applicable and permitted).
